One of the biggest things in endodontics in the last few years has been the development of bioactive sealers. The sealer is incredibly important in the proper healing of the situation.
Early in my career I had a conversation with a doctor who had a lot of clinical experience with endo and he explained to me why the sealer is so important. If you really think about it on a microscopic level, gutta percha, even if it is heated and plasticized, cannot easily flow into all the nooks and crannies of the canal system. Even properly cleaned and shaped canals are not uniform in shape. The job of the sealer is to get into all those little areas that are in the canal system and fill them. The idea is to eliminate voids and bacteria.
The doctor explained to me that he considered the sealer to be *more* important in healing than the gutta percha. He also told me that he looked at the cone as a piston that was used to create hydraulic pressure and drive the sealer into all of those little areas that remain after instrumentation. After that conversation, I began to pay a lot more attention to what type of sealer I used.
When bioactive sealers came on the market I felt that, anecdotally, I was getting better results. Post-op pain became much less common and healing seemed better. Now I'll be the first one to say that is completely my experience and NOT a scientific study... but I do feel these new bioactive sealers give me a better result.
That's why I was really excited to see this new sealer from Vista-Apex. The company has done a great job in the last 2-3 years with their Re-Gen products so when I saw they were bringing an endo sealer to market I was excited. I have not used the material yet. It just arrived in my office yesterday, but you can bet that I will be trying it on my next case. Give me some time to get a few cases done and I will report back to you here.
